For EtG, additional tests, or tests exceeding 10 panels, a total of 150mg of the specimen is advisable. It's advisable to weigh the specimen using a jeweler’s scale. If scalp hair isn’t available, a similar amount of body hair can be used. When referring to head hair, it strictly means scalp hair, whereas body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial steps necessary for integrating a sample into a lab’s system. This comprises ensuring the sample was correctly sealed and delivered, assigning a random LAN (Laboratory Accessioning Number), and finishing any extra data entry not managed by an electronic chain of custody system.

Screening comprises a swift preliminary check for substances of abuse. While Screening is a budget-friendly method to dismiss drug usage in most samples, a positive screening necessitates confirmation to stand up in a legal setting. Any samples marked presumptively positive in Screening need a secondary confirmation process.

Should a sample present a presumptive positive in the Screening phase, more hair is extracted from the original specimen and prepared for Extraction. Here, drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), rendering hair drug screening a complex procedure.

Positive Screening results undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing prior to confirmation, as necessary. The entire process, from Accessioning to Confirmation, is evaluated in accordance with CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
