Should scalp hair not be available, a similar volume of body hair is acceptable. When discussing head hair, it refers exclusively to scalp hair, whereas body hair includes all other types (such as facial and axillary).

Process Overview

The laboratory examination of a drug test involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially processed within a laboratory’s system. This step involves ensuring the sample is properly sealed and dispatched, assigning it a unique LAN (Laboratory Accessioning Number), and completing any further data entry outside of an electronic chain of custody system.

Screening provides an initial analysis for drugs of abuse. Although Screening is a cost-efficient method to dismiss drug usage in most samples, a positive result must undergo confirmation to be valid in legal contexts. All samples with a tentative positive in Screening require additional confirmation.

Should Screening reveal a presumptive positive, additional hair is taken from the original sample and readied for Extraction. In this phase, the process of extracting drugs from hair occurs at a much lower concentration compared to other methods (such as urine or oral fluid), rendering hair drug screening a more complex method to execute.

Confirmation for any positive screening outcome happens using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples initially indicating positive results are washed prior to final confirmation when needed. The entire lab procedure, from Accessioning to Confirmation, undergoes scrutiny under both the CAP (College of American Pathologists) guidelines for Hair and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
