A 1.5-inch segment of roughly 200 hair strands (comparable to a #2 pencil's thickness) nearest to the scalp provides 100mg of hair, the optimal specimen size for screening and verification. For EtG, additional tests, and tests exceeding 10 panels, 150mg is advised. We suggest measuring the sample with a jeweler’s scale. If scalp hair isn't available, a proportional amount of body hair can be collected. When discussing head hair, we mean scalp hair alone. Body hair includes other hair types (facial, axillary, etc.).

Process Overview

There are four primary steps in the lab processing of drug test outcomes: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of processing a sample into a lab’s system. This step confirms the sample's proper sealing and shipping, allocates a random LAN (Laboratory Accessioning Number), and covers any further data input not captured by an electronic chain of custody system.

Screening is a preliminary rapid check for drug abuse. Although Screening is a budget-friendly note to rule out drug use for most samples, a positive screen must be validated to be court-admissible. Samples testing presumptive positive during Screening require an additional confirmation.

If Screening indicates a presumptive positive, then more hair is taken from the original sample for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most complex to perform.

Any positive Screening results undergo Confirmation through GC/MS, GC/MS/MS or LC/MS/MS. All presumptively positive samples are washed before confirmation as necessary. The full lab procedure, from Accessioning to Confirmation, adheres to both the CAP (College of American Pathologists) Hair designation and accreditation per ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
