A sample size of 1.5 inches consisting of roughly 200 hair strands (similar in size to a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample size for both screening and confirmation. For EtG, additional testing, and/or tests greater than 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale for precise weighing of the sample. Should scalp hair be unavailable, an equivalent portion of body hair may be gathered. When discussing head hair, we mean scalp hair exclusively. Body hair pertains to all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of a sample within a laboratory's system. This includes confirming the sample’s proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not provided by an electronic chain of custody system.

Screening involves a preliminary quick check for drugs of abuse. While Screening serves as a cost-effective method to rule out drug usage in most samples, any positive results must be confirmed for legal admissibility. Samples that test presumptively positive during Screening necessitate a secondary confirmation.

Should a sample show a presumptive positive result during Screening, additional hair is extracted from the initial sample and prepared for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations than in other methodologies (e.g., urine or oral fluid), which contributes to the complexity of hair drug screening.

Confirmation of any positive Screening results is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation as deemed necessary. The comprehensive laboratory workflow from Accessioning to Confirmation adheres to the CAP (College of American Pathologists) Hair designation and maintains accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
