A 1.5-inch segment of approximately 200 strands (roughly the diameter of a #2 pencil) nearest the scalp will yield 100mg of hair, which is the optimal amount for evaluation and validation. For EtG, add-on tests, or screenings over 10 panels, 150mg of the sample is advised. We recommend using a jeweler’s scale to measure the sample. If scalp hair is absent, a comparable volume of body hair may be extracted. Reference to head hair signifies scalp hair specifically. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The processing of a lab drug test result encompasses four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab's system. This encompasses confirming that the sample was correctly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing necessary data input not provided by an electronic custody system.

Screening signifies an initial rapid assessment for substance abuse. Although Screening is an economical method to dismiss drug use across numerous samples, a positive result necessitates verification to be court-admissible. Samples that test presumptively positive in Screening require additional confirmation.

If a sample tests presumptively positive in Screening, additional hair is extracted from the initial sample and prepared for Extraction. In this phase, drugs are drawn from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening a challenging methodology.

Validation of any positive screening outcome is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed before confirmation as necessary. The comprehensive lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
