For EtG assessments, additional analyses, and/or tests exceeding 10 panels, a 150mg specimen is advisable. It's recommended to use a jeweler’s scale for weighing. In cases where scalp hair isn't available, equivalent body hair can be collected. By head hair, we specifically mean scalp hair. Body hair encompasses all other types (such as facial or axillary hair).

Process Overview

The laboratory assessment for drug tests involves four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where the sample is processed into the lab's system. This step verifies proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and includes any extra data entry absent from the electronic chain of custody system.

Screening represents a preliminary test for drug presence. Although cost-effective for eliminating drug use in most samples, any positive screen results must be verified for judicial acceptance. Samples showing a potential positive in Screening require additional confirmation.

For samples indicating a potential positive during Screening, additional hair is extracted and readied for the Extraction phase. Here, drugs are extracted at lower concentrations compared to other methods (e.g., urine or oral fluids), showcasing why hair testing is challenging.

Confirmed positive results undergo GC/MS, GC/MS/MS, or LC/MS/MS methods. Presumptive positives are cleaned before confirmation if necessary. The full lab process from Accessioning to Confirmation is scrutinized following CAP (College of American Pathologists) hair designation and aligns with ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
