A sample measuring 1.5 inches with around 200 hair strands (equivalent to the thickness of a #2 pencil) from the scalp yields 100mg, which is the optimum quantity for screening and validation. For EtG enhancements or tests beyond 10 panels, a specimen weighing 150mg is advised. It's suggested to utilize a jeweler’s scale for precise measurement. Should scalp hair be absent, a matching amount of body hair can be gathered. When discussing head hair, we specifically refer to scalp hair. Body hair encompasses all other types (such as facial and axillary hair).

Process Overview

The laboratory processing of a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning focuses on the initial integration of a sample into the laboratory’s system. This comprises checking that the sample is correctly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and finalizing any extra data input absent from an electronic chain of custody system.

Screening entails a preliminary assessment for substances of abuse. As a cost-effective method, Screening helps to discount drug use in most samples, yet a positive result must be validated for legal admissibility. Any samples preliminarily positive in Screening necessitate a further confirmation.

If a sample achieves a preliminary positive in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. During this phase, drugs are extracted from hair at concentrations lower than other methods (e.g., urine or oral fluid), making hair drug screening one of the most challenging methodologies to execute.

GC/MS, GC/MS/MS, or LC/MS/MS are employed to confirm any positive screening result. All samples preliminarily identified as positive are cleansed prior to confirmation, based on necessity. The entire process from Accessioning to Confirmation undergoes review per both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
