What it is, How it works

Hair analysis is increasingly recognized as an effective method for identifying drug and alcohol consumption. Hair offers an extended historical record of alcohol and drug intake by trapping markers within the hair fibers as they grow. When taken near the scalp, such a sample can present up to around 90 days of detection for alcohol and drugs. Hair collection is straightforward, challenging to tamper with, and simple to transport.

A 1.5-inch hair sample consisting of roughly 200 strands (about the width of a #2 pencil) near the scalp provides 100mg of hair, ideal for both screening and verification. For EtG, additional analyses, or tests exceeding 10 panels, 150mg of the specimen is advisable. It's suggested to weigh the sample with a jeweler’s scale. If scalp hair isn't available, an equivalent body hair amount may be gathered. In reference to head hair, it implies scalp hair exclusively. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial sample processing into a lab’s system. This comprises ensuring the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any additional data entries not covered by an electronic custody chain system.

Screening consists of a preliminary rapid assessment for drug abuse. Though Screening is a cost-effective method to rule out drug use in most samples, a positive result must be validated for legal admissibility. Any samples showing a presumptive positive in Screening necessitate additional confirmation.

If a sample appears positive during Screening, more hair is retrieved from the initial sample for Extraction preparation. Positive screening results are validated using GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptive positive samples are washed prior to confirmation when necessary. The entire laboratory process, from Accessioning to Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
