A 1.5-inch segment of roughly 200 hair strands (equivalent to the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the ideal sample for both screening and verification. For EtG, additional tests, and/or those exceeding 10 panels, 150mg of the sample is advised. We suggest measuring the sample on a jeweler’s scale. If scalp hair is unavailable, a similar quantity of body hair can be obtained. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The four critical steps involved in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the preliminary processing of a sample within a lab’s system. This phase includes verifying that the sample was effectively sealed and transported, allotting a random LAN (Laboratory Accessioning Number), and completing any supplementary data input absent from an electronic chain of custody system.

Screening constitutes an initial rapid check for substances of abuse. Although Screening is an economical method to negate drug use in most samples, a positive screen requires confirmation to be valid in court. Any samples that result in a presumptive positive in Screening necessitate a secondary confirmation.

Should a sample yield a presumptive positive during the Screening stage, additional hair is drawn from the original specimen and prepped for Extraction. Here, drugs are removed from hair at a much lower concentration than with other methods (e.g., urine or oral fluid), which explains why hair drug examination is the most challenging method to execute.

Verification of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if necessary. The entire laboratory sequence from Accessioning to Verification is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
