For EtG, add-ons, and tests exceeding 10 panels, it's suggested to obtain 150mg of the sample. It's advisable to use a jeweler’s scale for weighing. If scalp hair isn’t available, a similar quantity of body hair can be taken. When we mention head hair, it specifically refers to scalp hair, whereas body hair includes all other types (such as facial or axillary hair).

Process Overview

The primary steps in lab analysis of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the sample’s initial input into the laboratory's system. This process includes confirming the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input not provided by an electronic chain of custody.

Screening involves a preliminary examination to identify drugs of abuse. While Screening is cost-effective for eliminating drug usage in most samples, a positive result requires confirmation to be valid in legal situations. Every sample showing a presumptive positive in Screening must undergo further verification.

Should a sample test presumptive positive during Screening, additional hair is extracted from the original sample for the Extraction process. Here, drugs are retrieved from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), underlining why hair drug screening is particularly challenging.

Confirmation of any positive screening findings is executed via GC/MS, GC/MS/MS or LC/MS/MS. Before confirmation, all presumptive positive samples receive necessary washes. The full lab process from Accessioning to Confirmation receives review under both CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
