What it is, How it works

When harvested near the scalp, hair can indicate usage of alcohol and other substances over an estimated timeframe of three months. The collection process for hair is straightforward, resistant to tampering, and convenient for transport.

A 1.5-inch section consisting of roughly 200 strands, akin to the thickness of a #2 pencil, located closest to the scalp, offers 100mg of hair, which is optimal for screening and confirmatory purposes. For analyses involving EtG, additional panels, and/or tests exceeding 10 panels, it is advisable to obtain 150mg of the hair sample. We suggest using a jeweler’s scale to weigh the specimen. In the absence of scalp hair, an equivalent volume of body hair can be acquired. When addressing head hair, we mean scalp hair, while body hair encompasses other types (facial, axillary, and the like).

Process Overview

The principal stages in the laboratory evaluation of a drug test comprise Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial incorporation of a sample into the laboratory’s framework. This step verifies that the sample is correctly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and finalizes any supplemental data that is not provided through an electronic chain of custody.

Screening includes a preliminary swift evaluation for illicit substances. Though Screening is an economical approach to excluding drug usage in most samples, a positive screen necessitates affirmation to be valid in legal settings. Any samples that show a presumptive positive in Screening require secondary confirmation.

Should a sample indicate a presumptive positive during the Screening stage, additional hair from the original sample is gathered for the Extraction phase. In this segment, substances are extracted from hair at significantly lower concentrations than in alternative methods (for instance, urine or oral fluid), underscoring why hair drug testing is notably challenging.

Confirming a positive Screening result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all samples with a presumptive positive are cleansed as necessary. The entire laboratory procedure, from Accessioning to Confirmation, is rigorously assessed under both the CAP (College of American Pathologists) Hair credential and the ISO / IEC 17025 standard accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
