Hair collection is straightforward, relatively resistant to tampering, and easy to transport.

A 1.5-inch sample comprising roughly 200 hair strands (akin to the thickness of a #2 pencil) taken from the scalp region yields 100mg, which is optimal for screening and confirmation. For EtG assessments, additional screenings, and/or tests exceeding 10 panels, a sample of 150mg is recommended. It is advised to measure the specimen's weight using a jeweler’s scale. If scalp hair cannot be obtained, a proportional amount of body hair may be used. When mentioning head hair, it implies only scalp hair. Body hair encompasses other varieties (facial, axillary, etc.).

Process Overview

The drug testing laboratory processes involve four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into the laboratory infrastructure. This process includes ensuring the sample was appropriately sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing supplementary data entries absent from an electronic chain of custody system.

Screening entails a preliminary rapid assessment for drug presence. Despite Screening's cost-effectiveness in excluding drug presence in most samples, a confirmed positive screen is essential for legal admissibility. Samples showing a presumptive positive during Screening necessitate subsequent confirmation.

For samples that test presumptively positive during the Screening phase, additional hair is extracted from the initial sample for the Extraction phase. In this phase, drugs are extracted at much lower concentrations from hair than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive screening results occurs through GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive samples undergo washing before confirmation as necessary. The entire laboratory cycle, from Accessioning to Confirmation, adheres to both the CAP (College of American Pathologists) Hair designation and accreditation under ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
