A sample size of 1.5 inches containing approximately 200 hair strands (similar to a #2 pencil in thickness) nearest to the scalp will provide 100mg of hair, which is the perfect sample for both screening and confirmation. For EtG, additional tests, and/or assessments exceeding 10 panels, it is recommended to collect 150mg of the specimen. Utilizing a jeweler's scale to weigh the sample is advised. If no scalp hair is obtainable, an equivalent quantity of body hair can be taken. When referencing head hair, we mean only scalp hair. Body hair refers to any other hair forms (facial, axillary, etc.).

Process Overview

The primary four steps in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial processing of a sample into a lab's system. This step includes ensuring the sample was properly sealed and dispatched, assigning an arbitrary LAN (Laboratory Accessioning Number), and finishing any supplementary data entry absent in an electronic chain of custody system.

Screening is a basic preliminary check for substances of abuse. Even though Screening is a cost-efficient approach to eliminate drug use in the majority of specimens, a positive screen must be verified to be valid in legal proceedings. Any samples that are initially positive in Screening require a confirmatory evaluation.

If a sample tests positive in the Screening phase, additional hair is taken from the initial sample and prepped for Extraction. At this phase, drugs are extracted from hair at a notably lower concentration compared to other methods (e.g., urine or oral fluid), which is why conducting hair drug screening is the most intricate method.

Confirmation of any positive screen is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with an initial positive screen undergo washing before confirmation if needed. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair specification and the accreditation to ISO / IEC 17025 standards.
