What it is, How it works

Hair analysis has emerged as an effective means for identifying drug and alcohol consumption. Hair provides an extended timeline of substance use by embedding biomarkers within the strands as they grow. Collected near the scalp, a hair sample can offer a detection range of up to three months for alcohol and drugs. Hair collection is straightforward, not easily tampered with, and convenient for shipping.

A 1.5-inch section of about 200 hair strands, resembling the width of a #2 pencil, gathered closest to the scalp yields 100mg of hair, the optimal amount for testing. For EtG, additional panels, or tests surpassing 10 panels, 150mg is advisable. We suggest measuring the hair sample on a jeweler’s scale. If scalp hair isn't available, a similar volume of body hair can be collected. When mentioning head hair, it specifically refers to scalp hair. Body hair covers other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entering of a sample into the lab's system. This process includes validation that the sample was properly sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing extra data input if not provided by an electronic chain of custody.

Screening is a preliminary inspection for drugs. Although Screening is a cost-effective measure to rule out drug use in most cases, any positive screen must be confirmed for legal admissibility. Samples returning presumptive positives in Screening require follow-up confirmation.

For samples with a presumptive positive result from Screening, additional hair is extracted from the initial collection for further Extraction. In this stage, drugs are isolated from the hair at much lower levels compared to other methods (e.g., urine or oral fluid), making hair drug testing one of the most challenging methods to execute.

Any positive result in screening is validated using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleansed before confirmation as necessary. The entire laboratory method, from Accessioning to Confirmation, is scrutinized under both CAP (College of American Pathologists) Hair standards and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
