What it is, How it works

When taken near the scalp, hair samples can offer approximately a 3-month detection period for alcohol and other drugs. Hair is easy to collect, challenging to tamper with, and convenient to transport.

A 1.5-inch hair sample consisting of about 200 strands (similar in size to a #2 pencil) from the scalp delivers 100mg of hair, which is the perfect amount for screening and confirmation. For EtG, add-ons, and/or tests beyond 10 panels, it is advised to use 150mg of the specimen. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is unavailable, an equivalent amount of body hair can be used. When mentioning head hair, we only mean scalp hair. Body hair refers to any other hair types (e.g., facial, axillary).

Process Overview

The four key phases in laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a sample into a lab’s system, confirming the sample's seals and shipping accuracy, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not handled by an electronic chain of custody system.

Screening provides a preliminary check for drug abuse. Although Screening is an economical method to eliminate drug usage in the majority of samples, a positive screen must be verified to be legally defensible. Any samples that show presumptive positive results in Screening require secondary confirmation.

If a sample appears presumptively positive during the Screening phase, additional hair is taken from the original specimen for preparation in the Extraction phase. Here, drugs are drawn from hair at a significantly lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug screening is considered the most challenging methodology to execute.

Confirmation of any positive screening outcome is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The entire lab procedure, from Accessioning to Confirmation, is assessed by the standards of both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
