What it is, How it works

It provides an extensive record of substance use by embedding biomarkers within the strands of hair as they grow. When hair specimens are clipped close to the scalp, they can offer a detection time frame of up to around three months for both alcohol and drugs. Collecting hair is straightforward, complex to tamper with, and convenient to transport.

Gathering a 1.5-inch segment consisting of around 200 hair strands (approximating the thickness of a #2 pencil) near the scalp yields 100mg of hair, which is the optimal quantity for both screening and confirming results. For EtG, additional tests, and/or testing beyond 10 panels, a sample of 150mg is advisable. We suggest using a jeweler’s scale to measure the specimen. If scalp hair isn't an option, an equivalent quantity of body hair can be used. References to head hair imply scalp hair only, whereas body hair pertains to other hair types (facial, underarm, etc.).

Process Overview

There are four fundamental stages in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes initial processing into the lab's system. This step ensures the sample was sealed and transported correctly, assigns a random LAN (Laboratory Accessioning Number), and fills in any additional data entry missing in an electronic chain of custody system.

The Screening phase involves a quick preliminary test for substance abuse. Although Screening is a cost-effective means to eliminate drug usage in most samples, a positive screen must be validated for court admissibility. Any samples initially testing positive in Screening necessitate a secondary confirmation.

If a Screening stage test is presumptively positive, more hair is extracted from the original sample for preparation in the Extraction stage. In this process, drugs are extracted from hair at significantly lower concentrations than in other methods (such as urine or oral fluids); hence, hair drug screening is the most complex method to execute.

Confirmation of any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as necessary. The full laboratory procedure from Accessioning through Confirmation is examined under the CAP (College of American Pathologists) Hair designation and with accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
