What it is, How it works

Hair analysis is now recognized as an effective means for identifying drug and alcohol consumption. It offers a long-term record of substance use by capturing biomarkers within the hair strand as it grows. When hair samples are taken from near the scalp, detection of drugs and alcohol can extend to around three months. Hair collection is straightforward, relatively resistant to tampering, and easy to send for analysis.

A sample of hair that is 1.5 inches long and contains about 200 strands (roughly equivalent to a #2 pencil in diameter) near the scalp provides about 100mg of hair, which is optimal for screening and confirmation. For EtG, add-ons, or tests that exceed ten panels, 150mg of hair is suggested. It is advised to weigh the hair sample using a jeweler’s scale. If scalp hair is not available, the same amount of body hair can be collected. When mentioning head hair, it specifically refers to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The processing of a drug test result in the lab involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial handling of a sample into a lab’s system. This step ensures the sample was sealed and shipped appropriately, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional data entry not covered by an electronic chain of custody system.

Screening is a preliminary check for drugs of abuse. Although Screening is an economical method to negate drug use in most samples, a positive result must undergo confirmation for court admissibility. Samples that indicate a positive result in Screening require further confirmation.

If Screening suggests a positive result, additional hair is extracted from the sample and readied for Extraction. Here, drugs are isolated from the hair at much lower concentrations than other methods (such as urine or oral fluid), highlighting hair drug analysis as the most complex methodology to perform.

Confirmation of any positive Screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples go through a washing prior to confirmation as needed. Every step of the laboratory process, from Accessioning to Confirmation, is reviewed under the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
