For EtG, additional testing, or testing beyond 10 panels, a 150mg sample is advised. It's best to weigh the hair sample using a jeweler's scale. If scalp hair isn't available, an equivalent volume of body hair can be used. In this context, 'head hair' specifically refers to scalp hair, while 'body hair' covers all other types (such as facial, underarm, etc.).

Process Overview

The drug testing laboratory procedure primarily involves four steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary stage where the sample is registered within a lab's system. This step verifies the sample has been properly sealed and transported, assigns a Laboratory Accessioning Number (LAN) randomly, and fills in any missing details not included in an electronic chain of custody system.

Screening executes a quick assessment to identify drug abuse. Although Screening provides an economical means to exclude drug use in most samples, a positive find must be confirmed for legal standing. Samples that screen positive presumptively necessitate a follow-up confirmation.

If Screening suggests a positive result, additional hair is extracted from the stored sample and prepped for Extraction. During Extraction, the concentration of drugs retrieved from hair is much lower compared to alternative methods like urine or oral fluid, underscoring the complexity of hair drug testing.

Confirmation of any positive Screening find employs GC/MS, GC/MS/MS, or LC/MS/MS techniques. Any samples that initially test positive are washed as required prior to proceeding with confirmation. The complete lab processing chain from Accessioning through to Confirmation is conducted in compliance with both the CAP (College of American Pathologists) Hair guidelines and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
