A 1.5-inch section of roughly 200 hair strands (around the width of a #2 pencil) nearest to the scalp delivers 100mg of hair, which is the ideal quantity for evaluation and verification. For EtG, additional checks, and/or analyses over 10 panels, 150mg is advised. It's best to measure the sample with a jeweler’s scale. Should scalp hair not be available, a similar amount from body hair can be utilized. When discussing head hair, this pertains exclusively to scalp hair. Body hair encompasses all other varieties (facial, axillary, etc.).

Process Overview

The laboratory handling of a drug test outcome comprises four core steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning accounts for the initial inclusion of a sample into a lab's system. This includes confirming that the sample was correctly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and finalizing any further data input not available through an electronic chain of custody system.

Screening entails an initial rapid assessment for drugs of abuse. Screening serves as a budget-friendly method to dismiss drug usage in most samples; however, a positive screen necessitates confirmation for legal acceptance. Any samples that indicate a positive result during Screening necessitate an additional confirmation.

If a sample indicates a presumptively positive result during the Screening phase, more hair is taken from the original sample to prepare for Extraction. During this stage, drugs are extracted from hair at concentrations significantly lower than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging procedure to perform.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptively positive undergo washing before confirmation if required. The entire laboratory sequence from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair classification and the compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
