For EtG, additional tests, and/or those exceeding 10 panels, 150mg of the sample is advised. Use a jeweler’s scale to weigh the specimen. If scalp hair is not present, an equivalent amount of body hair can be used. The term head hair is specifically for scalp hair, while body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

Laboratory processing of a drug test involves four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning integrates the initial sample into a lab's system. It checks if the sample was sealed and sent correctly, generates a random LAN (Laboratory Accessioning Number), and performs any extra data entry absent in an electronic chain of custody system.

Screening performs a rapid assessment for drugs of abuse. Although Screening provides a cost-effective method to dismiss drug usage in most samples, positive results require confirmation for legal acceptability. Presumptive positives need secondary confirmation.

When a sample shows presumptive positivity during Screening, additional hair is extracted from the primary sample and readied for Extraction. Here, drugs are isolated from hair at significantly lower concentrations than with other techniques (e.g., urine, oral fluid); hence, hair drug evaluation is considered the most challenging to conduct.

To confirm any screening positives, methods like GC/MS, GC/MS/MS, or LC/MS/MS are employed. Presumptively positive samples are cleansed before confirmation, if necessary. From Accessioning to Confirmation, the entire laboratory procedure undergoes scrutiny under both the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
