What it is, How it works

Regarded as a potent tool for identifying substance use, hair testing captures a long-term history of substances, locking biomarkers within the strands of growing hair. When collected from near the scalp, hair can reflect a detection period spanning up to three months for both alcohol and drugs. Hair collection is straightforward, somewhat challenging to tamper with, and easy to transport.

A 1.5-inch segment of roughly 200 hair strands (the thickness of a #2 pencil) right next to the scalp will provide approximately 100mg of hair, which is the optimal sample for screening and confirming. For tests involving EtG, add-ons, or exceeding 10 panels, 150mg is suggested. Using a jeweler's scale to weigh the specimen is recommended. If scalp hair cannot be obtained, an equivalent amount of body hair may be gathered. By head hair, it specifically means scalp hair only. Body hair pertains to all other types (facial, axillary, etc.).

Process Overview

The laboratory process for drug testing comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial sample processing into the lab’s system. This step ensures the sample is properly sealed and shipped, allocates a random LAN (Laboratory Accessioning Number), and fills in any extra data entry missing from an electronic chain of custody system.

Screening offers an initial quick assessment for substance abuse. Although Screening is an economical method to dismiss drug usage in most cases, a positive screening necessitates confirmation for court admissibility. Any samples deemed presumptively positive in Screening need subsequent confirmation.

For a sample marked presumptively positive during Screening, additional hair is extracted from the primary sample and set up for Extraction. This process retrieves drugs from hair at much lower concentrations than other methods (like urine or oral fluid), making hair drug screening the most challenging method to conduct.

Confirmation for any positive screening outcome is done via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing prior to confirmation if needed. The entire lab process from Accessioning through Confirmation is assessed following both the CAP (College of American Pathologists) Hair designation and accreditation per ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
