For EtG, additional considerations, and/or evaluations surpassing 10 panels, a 150mg specimen is advisable. We suggest assessing the sample using a jeweler’s scale. Should scalp hair be lacking, an equivalent quantity of body hair may be gathered. "Head hair" specifically refers to scalp hair, whereas "body hair" encompasses all other types of hair (like facial and axillary hair).

Process Overview

The primary phases involved in laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory's framework. This step includes authenticating that the sample was properly sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry omitted by an electronic chain of custody system.

Screening comprises an initial brisk assessment for substance misuse. Though Screening offers a cost-efficient method to refute drug usage for the vast majority of samples, a positive screen necessitates confirmation to be legally acceptable. Any samples with provisional positive results during Screening require a secondary verification.

Upon a presumptive positive in the Screening phase, additional hair is retrieved from the original sample and processed for Extraction. During this phase, drugs are extracted from hair at considerably lower concentrations compared to other methodologies (such as urine or oral fluid), underscoring why hair drug screening is among the most intricate methodologies to execute.

Confirmation of any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as necessary. The entire lab process from Accessioning through Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
