What it is, How it works

When gathered near the scalp, hair can reveal up to a three-month period of drug and alcohol usage. Collection is straightforward, tampering is relatively difficult, and shipping is convenient.

A sample measuring 1.5 inches from approximately 200 strands (comparable to a #2 pencil's thickness) nearest the scalp yields 100mg of hair, which is optimal for testing procedures. For EtG, supplementary tests, and/or screenings beyond 10 panels, a specimen of 150mg is advisable. It is suggested to weigh the sample using a jeweler’s scale. In cases where scalp hair is unavailable, a comparable quantity of body hair may be utilized. When referring to head hair, we specifically mean scalp hair. Body hair encompasses other types such as facial and axillary hair.

Process Overview

The laboratory drug testing procedure comprises four critical stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the initial integration of a sample into the laboratory's system. This step includes ensuring the sample is sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and finishing any additional data entry not provided by an electronic chain of custody.

Screening administers an initial assessment for substance abuse. While it is a cost-efficient method to exclude drug consumption in most samples, a positive Screening result must be validated to be court-admissible. Any samples with presumptive positive results in Screening demand further confirmation.

When Screening indicates potential positives, more hair is retrieved from the initial sample for the Extraction process. Drugs are extracted from the hair at lower concentrations than in other testing methods (e.g., urine or oral fluids), which contributes to the challenge of hair drug analysis.

Positive Screening results undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive specimens are pre-washed prior to confirmation as required. The full laboratory process from Accessioning through Confirmation adheres to CAP (College of American Pathologists) Hair standards and ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
