A hair sample of 1.5 inches, consisting of about 200 strands or the size of a #2 pencil, near the scalp yields 100mg of hair, which is ideal for both screening and confirmation. For EtG, additional tests, or tests exceeding 10 panels, a specimen of 150mg is suggested. It’s advisable to weigh the specimen using a jeweler’s scale. If scalp hair is not accessible, an equivalent amount of body hair can be used. When we mention head hair, it strictly refers to scalp hair, whereas body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory process for a drug test result involves four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial sample processing into a lab system, ensuring the sample was sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and completing supplemental data entry missing from an electronic chain of custody system.

Screening provides a preliminary rapid assessment for drugs of abuse. While Screening is an economical method to initially rule out drug use in most samples, any positive result must be confirmed for legal admissibility. Samples testing presumptive positive in Screening need further confirmation.

If Screening indicates a presumptive positive, more hair is extracted from the initial sample for Extraction. In this phase, drugs are isolated from hair at a lower concentration compared to other methods (e.g., urine or oral fluid), which makes hair drug testing one of the most challenging methodologies.

Positive Screening results go through Confirmation using technologies like GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens are cleansed before confirmation if required. The full lab procedure from Accessioning to Confirmation is under scrutiny by both the CAP (College of American Pathologists) Hair standards and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
