What it is, How it works

A 1.5-inch lock consisting of about 200 hair strands (similar in size to a #2 pencil) near the scalp yields 100mg of hair, which is the preferred amount for both screening and confirmation. For EtG add-ons and/or tests beyond 10 panels, a specimen weight of 150mg is advisable. We suggest using a jeweler’s scale to measure the specimen. If scalp hair cannot be obtained, an equivalent portion of body hair might be used. Reference to head hair specifically pertains to scalp hair, while body hair encompasses all other types (such as facial, axillary, etc.).

Process Summary

The processing of a drug test outcome in a lab comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample enters a laboratory’s framework. This phase includes ensuring that the sample was correctly sealed and dispatched, allotting a random LAN (Laboratory Accessioning Number), and filling in any additional data entry not already covered by an electronic chain of custody.

Screening serves as a preliminary check for drug use. Although Screening is a cost-efficient strategy to dismiss drug use in most samples, any positive outcome requires confirmation to be valid in a legal scenario. Presumptively positive samples in Screening necessitate a secondary confirmation.

Should a sample test positive in the Screening phase, additional hair is extracted from the original specimen for the Extraction step. Here, drugs are isolated from hair at much lower levels than other methods (like urine or oral fluid), making hair drug testing one of the most complex techniques.

Confirmation of any positive findings from Screening is executed using GC/MS, GC/MS/MS or LC/MS/MS. All samples presumed positive undergo washing prior to confirmation if needed. The complete laboratory procedure from Accessioning to Confirmation is assessed under the CAP (College of American Pathologists) Hair designation and in accordance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
