What it is, How it works

Hair analysis has become recognized as an effective method for identifying use of drugs and alcohol. Hair captures a long-term record of alcohol and drug use by embedding biomarkers within the fibers of the developing hair strand. When harvested close to the scalp, hair can offer a detection period of up to around 90 days for alcohol and drugs. Hair is straightforward to gather, relatively challenging to tamper with, and easy to transport.

A 1.5-inch segment of roughly 200 strands of hair (comparable in size to a #2 pencil) nearest to the scalp will provide 100mg of hair, the optimal quantity for both screening and confirming. For EtG, add-ons, and/or evaluations beyond ten panels, 150mg of the sample is suggested. We advise measuring the sample on a jeweler’s scale. If scalp hair is not present, a comparable volume of body hair may be collected. When speaking of head hair, we mean scalp hair solely. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The four primary steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial processing of a sample into a lab’s system. This step confirms the sample was sealed and dispatched correctly, assigns a random LAN (Laboratory Accessioning Number), and finishes any extra data input not provided by an electronic chain of custody system.

Screening is a preliminary fast check for addictive substances. While Screening is a cost-efficient method to discount drug usage on most samples, a positive screen must be validated for court admissibility. Any samples that are presumptively positive during Screening require further confirmation.

If a sample registers as presumptively positive in the Screening phase, additional hair is taken from the original sample and prepared for Extraction. At this phase, drugs are extracted from hair at a lower concentration than other techniques (e.g., urine or oral fluid), making hair drug testing a challenging procedure to execute.

Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS or LC/MS/MS. All samples that are presumptively positive undergo a washing process prior to confirmation as needed. The full lab procedure from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
