A 1.5-inch length comprising around 200 hair strands (similar to a #2 pencil in width) from the scalp provides 100mg of hair, the optimal amount for both screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, a 150mg specimen is advised. We suggest measuring the specimen with a jeweler’s scale. If scalp hair is absent, an equivalent volume of body hair can be used. When discussing head hair, we mean scalp hair exclusively. Body hair encompasses all other types, like facial or axillary hair.

Process Overview

The laboratory processing of a drug test result consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into the laboratory’s workflow. It includes verifying that the sample is properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not supplied by an electronic chain of custody system.

Screening involves a preliminary check for substances of abuse. Although Screening provides a cost-efficient way to exclude drug use in most samples, a positive screen must be confirmed to be legally valid. Any samples that indicate potential positivity in Screening require a secondary confirmation.

Should a sample show potential positivity in the Screening phase, further hair is extracted from the initial sample and readied for Extraction, where drugs are extracted at a much lower concentration than other methods (like urine or oral fluid), highlighting why hair drug assessment is the most challenging methodology.

Confirmation of any positive screen is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with potential positivity are washed before confirmation as necessary. The end-to-end laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair specification and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
