Collecting a 1.5-inch snippet of approximately 200 hair strands, akin to a #2 pencil in size, near the scalp yields 100mg of hair, which is perfect for both screening and confirmation. For EtG, supplementary tests, and/or those exceeding ten panels, it is suggested to collect 150mg. We advise measuring the sample using a jeweler’s scale. If the scalp hair is insufficient, an equivalent amount of body hair can be used. In the context of head hair, we specifically mean scalp hair. Body hair encompasses all other hair forms (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result entails four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a lab’s database. This step includes ensuring proper sealing and shipping of the sample, assigning it a random LAN (Laboratory Accessioning Number), and completing any further data entry that an electronic custody chain system doesn’t cover.

Screening serves as an initial swift assessment for drug use. While Screening effectively rules out drug use in a majority of samples at a low cost, a positive screen necessitates confirmation for legal admissibility. Samples exhibiting presumptive positivity in Screening mandate a subsequent confirmation.

Should a sample be presumptively positive during Screening, more hair is extracted from the original sample for the Extraction process. Here, drugs are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), contributing to hair drug testing being the most challenging method to execute.

The Confirmation phase for any positive screening result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positivity are cleansed before confirmation as needed. The complete laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair standards and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
