For EtG, additional procedures, or tests exceeding 10 panels, it is advisable to gather 150mg of the sample. Using a jeweler’s scale to measure the specimen is recommended. In scenarios lacking scalp hair, a similar quantity of body hair can be used. When we discuss head hair, we specifically mean scalp hair, while body hair encompasses other types like facial or axillary hair.

Process Overview

The primary stages in laboratory drug test processing include Accessioning, Screening, Extraction, and Confirmation.

Accessioning focuses on integrating a sample into a laboratory’s system. It entails ensuring the sample's proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody system.

Screening performs an initial assessment for drugs of abuse. Although economical for excluding drug usage in most samples, a positive screen necessitates confirmation for court admissibility. Samples with presumptive positive results in Screening require a follow-up confirmation.

Presumptive positive samples from the Screening phase undergo further hair extraction from the original sample during Extraction. Given that drugs are retrieved from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), hair drug screening presents higher challenges.

Confirmation of any positive screen result utilizes GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are cleansed prior to confirmation as necessary. The complete laboratory process from Accessioning to Confirmation is reviewed per CAP (College of American Pathologists) Hair certification and ISO / IEC 17025 accreditation standards.
