What it is, How it works

Hair testing has established itself as an effective method for detecting drug and alcohol consumption. Hair can capture biomarkers within the fibers of the growing hair strand, offering a prolonged timeline of alcohol and other drug use. Collecting hair near the scalp allows for the detection window to extend to roughly 3 months for alcohol and other drugs. Hair collection is straightforward, resistant to tampering, and straightforward to transport.

Acquiring a 1.5-inch segment made up of approximately 200 hair strands (around the size of a #2 pencil) closest to the scalp yields 100mg, considered ideal for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a specimen weight of 150mg is advised. We suggest weighing the sample with a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair may be used. When we mention head hair, it is specific to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory method for processing a drug test result comprises four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial sample processing within a laboratory’s system. The procedure confirms that the sample was correctly sealed and transported, assigns a random Laboratory Accessioning Number (LAN), and completes any further data entry not provided by an electronic chain of custody system.

Screening is a preliminary, quick test for drugs of abuse. Even though Screening is a cost-efficient method to rule out drug use for the majority of samples, a positive screen necessitates confirmation to be valid in court. Any samples with a presumptive positive in Screening undergo additional confirmation.

If a sample shows a presumptive positive in the Screening phase, additional hair is extracted from the original specimen for the Extraction process. During Extraction, drugs are isolated from hair at a significantly lower concentration compared to other techniques (e.g., urine or oral fluid), which is why hair drug screening is the most complex method to execute.

Confirmation of any positive screening result is undertaken through GC/MS, GC/MS/MS or LC/MS/MS. All samples with presumptive positives undergo washing prior to confirmation when necessary. The full laboratory procedure from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation alongside accreditation to ISO / IEC 17025 standards.
