Recently, hair follicle drug tests have become a more popular method for detection. Employers, judicial systems, and addiction specialists often prefer it to urine testing. These tests help organizations with strict anti-drug policies, legal authorities, and those on probation. Hair analysis has emerged as a highly effective means for identifying the usage of drugs and alcohol. Hair serves as a comprehensive record of substance consumption, capturing biomarkers within the strands of hair as they grow. When sampled near the scalp, hair can reveal a detection period of up to around three months for alcohol and other substances. Hair is straightforward to collect, moderately resistant to tampering, and easy to transport.
A 1.5-inch section consisting of roughly 200 hair strands (comparable in size to a #2 pencil) near the scalp yields 100mg of hair, which is the preferred quantity for testing and verification. For EtG, additional tests, and/or more than 10 panels, 150mg of the sample is advisable. Using a jeweler’s scale is recommended for weighing the sample. If head hair is unavailable, an equivalent quantity of body hair can be used. When mentioning head hair, it is specifically about scalp hair, whereas body hair encompasses all other hair varieties (facial, axillary, etc.).
Process Overview
The testing process in the lab involves four critical stages: Accessioning, Screening, Extraction, and Confirmation.
Accessioning pertains to the initial incorporation of a sample into the lab's system, ensuring the sample was correctly sealed and delivered, assigning a unique LAN (Laboratory Accessioning Number), and addressing any additional data input that’s not covered by an electronic chain of custody system.
Screening is the preliminary evaluation for prevalent drugs. Though Screening effectively rules out substance use for the majority of samples, any positive findings must be authenticated for legal proceedings. Samples with presumptive positive results in Screening necessitate further confirmation.
Should a sample yield a presumptive positive during Screening, additional hair is extracted from the original sample for the Extraction process. Here, drugs are isolated from hair at concentrations much lower than other methods (e.g., urine or oral fluid), highlighting the complexity of performing hair drug tests.
Confirmation of any positive from screening is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The entire process, from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 accreditation standards.
