A 1.5-inch section consisting of approximately 200 strands near the scalp provides 100mg of hair, which is ideal for both screening and validation. For EtG, additional parameters, and/or more than 10-panel tests, 150mg of the specimen is advisable. It's recommended to measure the specimen using a jeweler’s scale. If scalp hair is unavailable, a similar quantity of body hair can be used. Mention of head hair pertains solely to scalp hair, whereas body hair encompasses other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four vital steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning denotes the initial phase of sample integration into a lab’s system. This stage involves ensuring the sample's proper sealing and shipping, assigning a unique LAN (Laboratory Accessioning Number), and completing any absent data entry not facilitated by an electronic chain of custody system.

Screening consists of a preliminary swift evaluation for drugs of misuse. Although it is a budget-friendly method to eliminate drug use in most cases, any sample yielding a positive screen must be verified to be legally valid. Samples that test presumptively positive during Screening require further confirmation.

When a sample tests presumptively positive during Screening, additional hair from the original sample is processed for Extraction. At this juncture, drugs are isolated from hair at significantly lower concentrations than with other methods (such as urine or oral fluid), underscoring why hair drug analysis is among the most intricate testing methods.

Confirmed results from positive screenings are obtained using GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptive positive samples are cleansed before confirmation as necessary. The entire lab procedure from Accessioning to Confirmation is examined under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
