A 1.5-inch strand consisting of about 200 hairs (similar in size to a #2 pencil) nearest the scalp yields 100mg, an ideal amount for evaluation and verification tests. For EtG, additional checks, or for tests exceeding 10 panels, a 150mg sample is advised. Assessing the sample using a jeweler’s scale is suggested. If scalp hair is deficient, an equivalent quantity of body hair might be utilized. When referring to head hair, it specifically means scalp hair. Body hair includes all other hair variants (facial, axillary, etc.).

Process Overview

The laboratory processing for a drug test result includes four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary handling of a specimen in a lab's system. Verification that the specimen was correctly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any required data entry absent from an electronic custody chain are part of this stage.

Screening provides an initial swift assessment for drug abuse. Though cost-effective for eliminating drug usage in most instances, a positive result requires subsequent confirmation for court validity. Any sample yielding presumptive positive results in Screening necessitates additional confirmation.

For presumptively positive samples in Screening, additional hair is extracted from the original sample for Extraction preparation. Here, drug extraction from hair occurs at a lower concentration than in alternate methodologies (such as urine or oral fluids), which makes hair drug analysis a more complex procedure.

GC/MS, GC/MS/MS, or LC/MS/MS techniques are used for confirming any positive screen results. Before confirmation, all presumptive positives undergo washing as essential. From Accessioning to Confirmation, the laboratory process is analyzed under both CAP (College of American Pathologists) Hair standards and ISO / IEC 17025 accreditation.

Manuscripts in this section present advancements in forensic toxicology, and have explored the challenges posed by emerging designer drugs and strategies to combat their proliferation. The pitfalls encountered when the exogenous/endogenous nature of a substance is questioned were presented and discussed. The special issue presents also manuscripts that delve into the ethical implications of drug testing in drug facilitated sexual assaults.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
