A 1.5-inch portion of around 200 hair strands (similar in size to a #2 pencil) from the scalp yields 100mg of hair, which is the optimal quantity for screening and confirmation. For EtG, additional tests, and/or those exceeding 10 panels, 150mg of sample is advisable. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is absent, a comparable amount of body hair can be used. By head hair, we mean scalp hair alone. Body hair includes all other hair types (beard, armpit, etc.).

Process Overview

The laboratory processing of a drug test encompasses four core steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial sample processing within a lab's system. This step comprises ensuring the sample was appropriately sealed and shipped, generating a random LAN (Laboratory Accessioning Number), and accomplishing any supplementary data entry needed that isn't covered by an electronic chain of custody system.

Screening involves a preliminary swift review for drugs of abuse. Screening is an economical method for excluding drug use in most cases, yet a positive outcome requires verification to be court-admissible. Samples that appear positive in Screening need a subsequent confirmation.

Should a sample seem positive during the Screening phase, further hair is extracted from the primary specimen and set for Extraction. At this juncture, drugs are extracted from hair at significantly reduced concentrations than through other methods (e.g., urine or saliva), hence hair drug screening poses the utmost complexity.

Confirmation of a positive result from Screening is via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumed positive results undergo washing as required before confirmation. The comprehensive lab process from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
