What it is, How it works

Hair analysis is now viewed as an effective method for detecting drug and alcohol consumption. Hair offers a comprehensive record by preserving biomarkers within the strands. When sourced near the scalp, hair can present roughly a 3-month timeframe for alcohol and other narcotics. Collecting hair is straightforward, tampering is challenging, and transportation is straightforward.

Collecting an approximately 1.5-inch section of around 200 hair strands (equivalent to the girth of a #2 pencil) adjacent to the scalp yields 100mg of hair, the perfect amount for screening and verification tests. For EtG, additional tests, and/or panels exceeding 10, a sample of 150mg is suggested. We advise utilizing a jeweler’s scale to measure the specimen. If scalp hair is not available, an equivalent weight of body hair may be used. When discussing head hair, we refer solely to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test result encompasses four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase where a sample is logged into a lab’s system. This step includes verifying the sample’s seal and shipping, allocating a random LAN (Laboratory Accessioning Number), and finalizing any extra data input absent in an electronic chain of custody system.

Screening conducts an initial assessment for drugs of abuse. Screening is economical for discarding drug usage in numerous samples, yet a positive result requires confirmation for legal validation. Samples with a presumptive positive in Screening necessitate a follow-up verification.

For samples testing positive in Screening, additional hair is extracted and laid out for Extraction. In this procedure, drugs are isolated from hair at significantly lower concentrations than other methods (such as urine or saliva), which accounts for the difficulty in performing hair drug analysis.

Positive screening confirmations are executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples identified as presumptive positive undergo a wash before confirmation as necessary. From Accessioning to Confirmation, the full laboratory workflow is evaluated under both the CAP (College of American Pathologists) Hair guidelines and in compliance with ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
