What it is, How it works

When gathered near the scalp, hair can present nearly a 3-month period for identifying alcohol and other drugs. Hair is easy to gather, somewhat tricky to tamper with, and straightforward to send.

A 1.5-inch sample of around 200 hair strands (resembling a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal quantity for screening and confirmation. For EtG, add-ons, and/or evaluations exceeding 10 panels, 150mg of the sample is advisable. We suggest measuring the sample using a jeweler’s scale. If scalp hair is inaccessible, an equivalent amount of body hair may be used. When we mention head hair, we mean scalp hair alone. Body hair refers to all other hair (facial, axillary, and so on).

Process Overview

The laboratory processing of drug test results involves four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning revolves around the preliminary processing of a sample into a laboratory system. This includes ensuring the sample was sealed and shipped correctly, assigning it a random LAN (Laboratory Accessioning Number), and completing any further data entry not supplied by an electronic chain of custody system.

Screening offers an initial rapid assessment for drugs of abuse. While Screening is economically efficient for ruling out drug usage on most samples, a positive screen must be verified to be permissible in legal settings. Any samples suggestively positive during Screening require further confirmation.

If a sample appears presumptively positive during Screening, additional hair is extracted from the initial specimen for Extraction. At this stage, drugs are isolated from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug testing the most challenging method to conduct.

Confirmation of any positive screening result is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed before confirmation if needed. The entire laboratory process from Accessioning to Confirmation is evaluated based on both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
