What it is, How it works

Hair offers an extended history of alcohol and other substances by capturing biomarkers within the fibers of the growing hair strand. When taken near the scalp, hair can offer up to a roughly 3-month detection timeframe for alcohol and drugs. It is straightforward to collect, relatively hard to tamper with, and easy to transport.

A 1.5-inch segment containing around 200 strands of hair (approximately the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample for analysis and verification. For EtG, additional tests, and/or examinations beyond 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale to measure the specimen. If scalp hair is not available, a comparable amount of body hair should be gathered. When mentioning head hair, we are specifically referencing scalp hair. Body hair denotes all other hair forms (facial, axillary, etc.).

Process Overview

The four pivotal stages in the laboratory analysis of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial sample processing into a lab’s system. This involves ensuring the sample was sealed and dispatched properly, assigning a unique LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent from an electronic custody chain system.

Screening entails a preliminary rapid check for drug abuse. While Screening offers a cost-efficient method to negate drug use in most samples, any positive screen must be validated to be permissible in a legal setting. Samples presumed positive in Screening necessitate an additional confirmation.

Should a sample be deemed presumptively positive during the Screening phase, additional hair is sourced from the original specimen for Extraction. In this phase, drugs are retrieved from hair at significantly lower concentrations compared to other techniques (e.g., urine or oral fluid), which is why hair drug screening is the most intricate technique to execute.

Confirmation of positive screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo a wash prior to confirmation as necessary. The complete laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair criteria and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
