What it is, How it works

Hair analysis has emerged as an effective method for identifying drug and alcohol consumption. Hair captures a prolonged history of alcohol and drug usage by securing biomarkers within the strands' fibers as they grow. When gathered near the scalp, hair can provide a detection period for alcohol and drugs that spans up to about three months. Hair is easy to collect, somewhat complex to tamper with, and simple to send.

A 1.5-inch sample comprising roughly 200 hair strands (approximately the thickness of a #2 pencil) nearest the scalp will yield 100mg of hair, which is the preferred sample for testing and verification. For EtG, enhancements, and/or tests exceeding 10 panels, 150mg of the sample is advisable. We suggest measuring the sample with a jeweler’s scale. If no scalp hair is available, an equivalent quantity of body hair may be gathered. When discussing head hair, we mean scalp hair exclusively. Body hair includes any other hair types (facial, underarm, etc.).

Process Overview

The principal stages involved in the laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the preliminary processing of a sample into a lab's system. It entails ensuring the sample was appropriately sealed and sent, allocating a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent in an electronic chain of custody system.

Screening constitutes an initial rapid assessment for drugs of misuse. Though Screening offers a cost-saving method to dismiss drug use in most samples, a positive screen necessitates confirmation to be valid in court. Any samples that are tentatively positive in Screening demand a second confirmation.

If a sample is tentatively positive during the Screening step, additional hair is taken from the primary specimen and readied for Extraction. In this stage, drugs are drawn from hair at concentrations much lower than other approaches (e.g., urine or oral fluid), which makes hair drug screening the most intricate methodology to conduct.

Confirmation of any positive Screening outcome is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All tentatively positive samples are cleansed before confirmation if necessary. The full laboratory sequence from Accessioning through Confirmation is reviewed under both the CAP (College of American Pathologists) Hair classification and the approval to ISO / IEC 17025 standards.

Advantages of hair drug testing:

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
