Obtaining a 1.5-inch segment of around 200 hair strands (approximately the thickness of a #2 pencil) closest to the scalp yields 100mg, optimal for both screening and confirmation processes. For EtG tests, additional testing, and/or those beyond 10 panels, 150mg is suggested. It's advised to measure the sample using a jeweler's scale. If head hair isn't available, an equivalent amount of body hair is acceptable. "Head hair" pertains to scalp hair solely, while "body hair" encompasses all other types (like facial, armpit, etc.).

Process Overview

The drug test result processing in a lab involves four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first stage where a sample is logged into a lab's system. This includes confirming the sample's proper sealing and shipping, assigning a unique LAN (Laboratory Accessioning Number), and entering any supplemental data not pre-loaded by an electronic chain of custody system.

Screening is an initial rapid assessment for drug abuse. Although Screening is a budget-friendly means to dismiss drug use in most samples, any positive Screening must be verified for legal acceptance. Samples with presumptive positives in Screening necessitate further confirmation.

When a sample tests presumptively positive during Screening, additional hair from the original specimen is prepared for Extraction. Here, drugs are withdrawn from hair at much lower levels than in other methods (like urine or saliva), highlighting the challenge of hair drug screening.

Verdict of any positive screening is conducted with GC/MS, GC/MS/MS or LC/MS/MS technology. Any samples with presumptive positives undergo a washing prior to confirmation when necessary. The entire lab process, from Accessioning to Confirmation, is reviewed under both the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
