For EtG, add-ons, and/or panels exceeding 10 tests, a specimen size of 150mg is advised. It is advisable to use a jeweler’s scale for weighing the specimen. In cases where scalp hair cannot be sourced, an equivalent quantity of body hair may be utilized. References to head hair specify scalp hair exclusively. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory handling of a drug test outcome principally involves Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed within the laboratory’s framework. This phase includes confirming the sample’s seal and shipment were managed correctly, assigning a random LAN (Laboratory Accessioning Number), and entering any additional data not included in an electronic chain of custody system.

Screening represents an initial, swift assessment for drug misuse. While Screening presents a cost-efficient method to exclude drug use in most samples, a positive result necessitates confirmation for legal admissibility. Any samples giving a presumptive positive outcome in Screening require a further confirmation step.

Should a sample yield a presumptive positive in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. This phase involves extracting drugs from hair at significantly lower concentrations than other methods (e.g., urine or oral fluids), underscoring the complexity of hair drug screening versus other methodologies.

Confirmation of any positive Screening findings is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples are duly cleansed as required. The complete laboratory sequence from Accessioning to Confirmation is assessed in accordance with both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.
