Hair is straightforward to collect, hard to tamper with, and simple to transport.

A sample of 1.5 inches, consisting of around 200 strands of hair (equivalent to the size of a #2 pencil) near the scalp, will yield 100mg of hair, which is the ideal sample for analysis and verification. For EtG, add-ons, or tests exceeding 10 panels, it is advisable to use 150mg of the sample. It's suggested to weigh the sample using a jeweler's scale. Should scalp hair be inaccessible, an equivalent quantity of body hair can be utilized. When we mention head hair, we are specifically referring to scalp hair. Body hair covers all other hair variants (facial, axillary, etc.).

Process Overview

The key steps in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the primary processing of a sample within a lab’s system. This stage includes verifying proper sealing and shipment of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing necessary data entry not covered by an electronic chain of custody system.

Screening serves as an initial, rapid assessment for drugs of abuse. Although it is a cost-efficient method to rule out drug use for most samples, a positive result from Screening requires confirmation for court admissibility. Any samples flagged as presumptively positive in Screening merit secondary verification.

If a sample is identified as presumptively positive during the Screening phase, additional hair is extracted from the original sample to prepare for Extraction. This phase involves extracting drugs from hair at considerably lower concentrations compared to other techniques (such as urine or oral fluid), explaining why hair drug screening is among the most challenging to perform.

Confirmation of any positive Screening result is done via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as necessary. The full lab process from Accessioning to Confirmation is examined under the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
