What it is, How it works

Hair provides an extended history of alcohol and substances by embedding biomarkers within the fibers of the hair as it grows. When gathered close to the scalp, hair can reveal a detection span of up to around three months for alcohol and drugs. Hair collection is straightforward, moderately tough to tamper with, and facilitates easy shipping.

A 1.5-inch hair sample consisting of approximately 200 strands (similar to a #2 pencil) nearest the scalp provides 100mg of hair, which is the optimum specimen for analysis and validation. For EtG enhancements, and/or testing beyond 10 panels, a specimen of 150mg is advised. We suggest measuring the specimen using a jeweler’s scale. If scalp hair is unavailable, a comparable amount of body hair can be collected. When mentioning head hair, we refer explicitly to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

There are four primary stages in the lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary processing of a sample into a lab's system. This ensures the sample is appropriately sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and fills in any further data not supplied by an electronic chain of custody system.

Screening entails an initial rapid assessment for drugs of abuse. Although Screening is an economical approach to eliminate drug usage in most samples, a positive screen must be confirmed for court admissibility. Samples that test presumptively positive in Screening require secondary verification.

For samples that are presumptively positive during Screening, additional hair is sourced from the original sample and prepared for Extraction. In this phase, drugs are extracted from hair at a much lesser concentration compared to other techniques (e.g., urine or oral fluid), highlighting the complexity of performing hair drug screening.

Confirmation for any positive Screening result occurs through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo a wash prior to confirmation if necessary. The entire lab procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and certification to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
