What it is, How it works

Hair collection is straightforward, challenging to tamper with, and easily transportable.

A 1.5-inch portion consisting of roughly 200 strands (comparable in size to a #2 pencil) closest to the scalp yields 100mg of hair, which is optimal for both initial tests and confirmations. For EtG, additional protocols, and/or tests exceeding 10 panels, a 150mg sample is advisable. For accuracy, use a jeweler’s scale for weighing. Should scalp hair be inaccessible, a similar quantity of body hair may be used. "Head hair" specifically refers to scalp hair. "Body hair" pertains to all other hair types (facial, axillary, etc.).

Process Overview

Drug test processing in laboratories involves four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a sample into a lab's system, ensuring the sample was correctly sealed and transported, assigning a random Laboratory Accessioning Number (LAN), and completing any extra data entry missing from an electronic custody system.

Screening is an initial efficient check for drugs. Although Screening is cost-efficient for dismissing drug usage in most cases, a positive outcome must be confirmed to be valid in legal situations. Any samples with presumptive positives in Screening must undergo a secondary confirmation.

If a Screening stage sample tests presumptively positive, additional hair is extracted from the primary specimen and prepared for Extraction. Drugs are removed from hair in this stage at much lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging practice to execute.

Confirming any positive Screening results involves using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleaned before confirmation if necessary. The complete laboratory procedure, from Accessioning to Confirmation, is audited under the CAP (College of American Pathologists) Hair category and adheres to ISO / IEC 17025 certification.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
