For EtG, additional tests, and/or more than 10-panel tests, a specimen of 150mg is advised. It is best to weigh the sample using a jeweler’s scale. If scalp hair cannot be obtained, an equivalent amount of body hair can be used. When mentioning head hair, it specifically refers to scalp hair only. Body hair encompasses other categories like facial and axillary hair.

Process Overview

Laboratory processing of a drug test result includes four major phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial handling of a sample within a laboratory's framework. This phase entails confirming the sample’s proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any further data input not encompassed by an electronic chain of custody system.

Screening is a preliminary, efficient evaluation for illicit substances. While Screening provides a cost-effective method to negate drug presence for most samples, any positive result necessitates verification for legal validity. Samples presumptively positive in Screening require a subsequent confirmation.

Should a sample register as presumptively positive during Screening, more hair is extracted from the original sample and prepped for Extraction. Within this phase, substances are extracted from hair at significantly lower concentrations than those found in alternative techniques (e.g., urine or oral fluid), which is why hair drug testing is the most challenging methodology.

Confirmation for any positive screens involves GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives undergo washing before confirmation as necessary. The entire laboratory procedure from Accessioning to Confirmation is conducted under the oversight of both the CAP (College of American Pathologists) Hair identification and the standards accredited by ISO / IEC 17025.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
