What it is, How it works

A sample of 1.5 inches with around 200 hair strands (approximately the width of a #2 pencil) cut nearest to the scalp provides 100mg, which is the optimal quantity for initial testing and confirmation. For EtG, additional tests, and/or panels exceeding 10, a specimen amount of 150mg is advised. Using a jeweler’s scale to weigh the hair is suggested. If scalp hair cannot be obtained, a comparable amount of body hair may be used. When head hair is mentioned, it specifically refers to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

Four primary phases in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, a sample is initially processed into the lab system. This step involves confirming that the sample was sealed and transported correctly, allocating a random LAN (Laboratory Accessioning Number), and completing any additional data entry missing from the electronic chain of custody system.

Screening involves an initial rapid assessment for drug abuse. Though Screening is cost-effective for excluding drug use in most samples, positive screens must be confirmed to be admissible in court. Presumptively positive Screening samples necessitate further confirmation.

If a sample tests presumptively positive during Screening, more hair is extracted from the initial sample for the Extraction phase. In this phase, drugs are extracted from the hair in much lower concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug screening one of the most challenging methods to execute.

Confirmation of a positive screen result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptively positive are washed before confirmation as necessary. The comprehensive lab process from Accessioning to Confirmation undergoes review in line with both the CAP (College of American Pathologists) Hair designation and accreditation to ISO/IEC 17025 standards.
