Collecting a 1.5-inch segment of roughly 200 hair strands (about the thickness of a #2 pencil) nearest the scalp yields 100mg, the optimal amount for analysis and confirmation. For EtG, add-ons, or tests exceeding 10 panels, 150mg is advised. Utilizing a jeweler’s scale for weighing the sample is recommended. In the event scalp hair isn't accessible, an equivalent quantity of body hair can be used. References to head hair specifically mean scalp hair, while body hair pertains to all other types (like facial, axillary, etc.).

Process Overview

The primary stages in the lab process of a drug test include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves logging the sample into a lab's system, ensuring it was correctly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and adding any extra data not supplied by an electronic custody system.

Should a sample be positive after Screening, additional hair is extracted from the provided sample and prepped for Extraction. Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing before confirmation as required. The comprehensive lab process, from Accessioning to Confirmation, is assessed under the CAP (College of American Pathologists) Hair designation alongside accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
