The key advantage is the extended detection period of up to 90 days, making it hard to deceive; however, for drugs used within the last five days, urine tests remain most precise.

What it is, How it works

Hair analysis is now viewed as an effective method for identifying drug and alcohol consumption. By embedding biomarkers within the strands of hair as they grow, it chronicles a long-term record of alcohol and drug use. When harvested near the scalp, hair can reveal signs of alcohol and drug presence over an approximately 3-month period. Hair collecting is straightforward, challenging to tamper, and convenient for mailing.

A 1.5-inch snippet of around 200 hair strands (similar to a #2 pencil thickness) nearest to the scalp yields 100mg, an optimal quantity for testing and confirmation. For EtG, additional analyses, and/or evaluations surpassing 10 panels, a 150mg specimen is advised. We suggest measuring the specimen using a jeweler's balance. If head hair is not accessible, an equivalent quantity of body hair should be gathered. By head hair, we mean only scalp hair. Alternatively, body hair includes any other types such as facial or axillary.

Process Overview

Four essential steps are taken in the lab processing of a drug test: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a laboratory’s database. It includes verifying the sample’s sealed and proper shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry not covered by an electronic chain of custody system.

Screening is a preliminary rapid check for drugs of abuse. While Screening is a cost-effective strategy to exclude drug usage in most samples, a positive result in Screening requires confirmation to qualify for legal evidence. Samples with presumptive positive results in Screening necessitate a secondary verification.

If a Screening result is presumptively positive, additional hair is taken from the original sample and prepped for Extraction. In this phase, drugs are separated from the hair at much lower concentrations than other methods (ex. urine or oral fluids), highlighting the complexity of hair drug screening.

Positive screening results are corroborated via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positivity are cleansed prior to confirmation as required. The entire lab workflow from Accessioning to Confirmation is scrutinized according to the CAP (College of American Pathologists) Hair designation and adherence to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
