For EtG, additional tests, and/or tests exceeding 10 panels, gathering 150mg of the specimen is advisable. It is suggested to measure the specimen using a jeweler’s scale. In the absence of scalp hair, a similar quantity of body hair can be used. When we mention head hair, we specifically refer to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result comprises four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial entry of a sample into a lab’s system. This process includes ensuring that the sample was properly sealed and delivered, assigning a unique LAN (Laboratory Accessioning Number), and completing any missing data entry not available through an electronic chain of custody system.

Screening encompasses a preliminary overview for drugs of abuse. Though Screening is a cost-effective method to exclude drug use in most samples, a positive result needs confirmation for legal admissibility. Any samples that show presumptive positivity in Screening necessitate further confirmation.

Should a sample show presumptive positivity at the Screening phase, additional hair is extracted from the initial sample for the Extraction process. At this phase, drugs are isolated from hair at significantly lower concentrations than in other techniques (e.g., urine or oral fluid), highlighting why hair drug testing is notably challenging.

Confirmation for any positive screen outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Any samples with presumptive positivity undergo washing as necessary prior to confirmation. The entire laboratory operation, from Accessioning to Confirmation, is supervised under both the CAP (College of American Pathologists) Hair designation and adherence to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
