What it is, How it works

Hair testing is now regarded as an impactful means for identifying drug and alcohol consumption. Hair gives a lengthy history of alcohol and other substances by ensnaring biomarkers within the fibers of its growing strands. When taken near the scalp, hair can offer up to an estimated 3-month detection window for alcohol and other substances. Hair is straightforward to collect, fairly resistant to tampering, and simple to transport.

A 1.5-inch section of approximately 200 hair strands (around the width of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the best sample for both screening and verification. For EtG, add-ons, and/or assessments exceeding 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is not accessible, a comparable volume of body hair can be gathered. When mentioning head hair, we mean scalp hair only. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The primary steps in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample into a lab's system. This consists of confirming that the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data not included by an electronic chain of custody system.

Screening represents an introductory rapid assessment for drug use. Although Screening is an economical method to exclude drug usage for most samples, a positive screen must be confirmed to be legally viable. Any samples that are presumed positive in Screening must have a secondary validation.

If a sample appears presumptive positive in the Screening phase, extra hair is sourced from the initial specimen and readied for Extraction. In this phase, substances are withdrawn from hair at a much lower concentration compared to other methods (e.g., urine or oral fluid), hence hair drug testing is the most challenging method to perform.

Confirmation of any positive Screening result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed prior to confirmation as required. The complete laboratory process from Accessioning to Confirmation is reviewed adhering to both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
