For EtG, supplemental tests, and/or panels exceeding 10, 150mg of hair is advised. We suggest using a jeweler’s scale for weighing. If head hair isn’t available, a similar amount of body hair can be taken. By head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The key phases in lab processing of a drug screening result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves integrating a sample into a laboratory’s workflow. This process includes checking the sample’s seal and correct shipping, assigning a LAN (Laboratory Accessioning Number), and performing any extra data entry absent in an electronic chain of custody system.

The Screening phase offers a preliminary check for substances. Although it's a cost-effective method to discount drug use in most samples, a confirmed positive screen is necessary for legal purposes. Any samples that test positive in Screening require further confirmation.

When a sample tests positive in the Screening, more hair is drawn from the original sample and prepped for Extraction. Drugs are extracted at much lower concentrations than in other methods (e.g., urine, oral fluid), making hair drug testing the most challenging to conduct.

Verification of positive screening findings is done using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing as needed before confirmation. The complete lab process, from Accessioning to Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair guideline and the ISO / IEC 17025 standard.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
